{"id":6366,"date":"2019-10-11T11:33:00","date_gmt":"2019-10-11T06:33:00","guid":{"rendered":"https:\/\/drmohans.com\/?p=6366"},"modified":"2024-11-27T14:05:06","modified_gmt":"2024-11-27T14:05:06","slug":"diabetic-retinopathy","status":"publish","type":"post","link":"https:\/\/drmohans.com\/backend\/diabetic-retinopathy\/","title":{"rendered":"Diabetic Retinopathy"},"content":{"rendered":"\r\n<p class=\"wp-block-paragraph\"><strong>Dr. V Mohan<\/strong><\/p>\r\n\r\n\r\n\r\n<p class=\"wp-block-paragraph\"><strong>Chairman, Dr. Mohan\u2019s Diabetes Specialities Centre<\/strong><\/p>\r\n\r\n\r\n\r\n<p class=\"wp-block-paragraph\">Of the various complications of diabetes, visual\r\nimpairment is perhaps the most feared by people with diabetes. Diabetic\r\nindividuals are much more prone to visual disability than non-diabetic\r\nindividuals. The impact of visual impairment among diabetic individuals has\r\nprofound implications for the person affected and society as a whole. Visual\r\ndisability significantly affects the quality of life and increases the risk of\r\ninjury. The most distressing effects of diabetes in the eye with regard to\r\nvisual impairment are in the retina (inner layer of the eye) called as diabetic\r\nretinopathy. Preventive approaches to diabetic retinopathy can be categorized\r\ninto three levels, the first is <strong>Primary\r\nPrevention <\/strong>of retinopathy. This is an early approach where early detection\r\nand appropriate treatment are the cornerstones for delaying the onset of the\r\ndiabetic retinopathy symptoms by good control of blood sugar, blood pressure\r\nand lipids. It is mandatory that every type 2 diabetic individuals should have\r\na retinal examination at the time of diagnosis of diabetes and for type 1\r\ndiabetic subjects retinal examination should be done 5 years after diagnosis\r\nand every year thereafter.<\/p>\r\n\r\n\r\n\r\n<p class=\"wp-block-paragraph\">Diabetic retinopathy classification can be\r\nbasically divided into two types \u2013 non-proliferative diabetic retinopathy and\r\nproliferative diabetic retinopathy. Non-proliferative diabetic retinopathy is\r\nthe early stage of the disease and at this stage there are small balloon like\r\nsacs (out pouching of capillaries) called as micro aneurysms. In the next stage,\r\nthese sacs leak fluid and blood into the retina. During this stage, no sight\r\nloss is present. However, once diabetic retinopathy progresses, prevention of\r\nthe same would form the second approach or <strong>Secondary\r\nPrevention<\/strong>. As part of secondary prevention one can prevent the progression\r\nof this stage by advocating good control of diabetes in addition to documenting\r\nthe retinal changes by colour retinal photography. This is made possible by the\r\nadvent of the digital system of colour photography, which makes it easier for\r\nthe eye specialist to explain to the subjects about the retinal condition. As a\r\nfollow up, dilated retinal examination should be done more frequently at this\r\nstage of the disease. As individuals may not have any diabetic retinopathy\r\nsymptoms, life-long evaluation for retinopathy is a valuable and necessary\r\nstrategy. It has been estimated that in whom eye examination had not previously\r\nbeen performed, 50% subsequently had visual disability due to diabetic\r\nretinopathy.<\/p>\r\n\r\n\r\n\r\n<p class=\"wp-block-paragraph\">The two sight threatening forms of diabetic\r\nretinopathy classification are diabetic macular edema (DME) and Proliferative\r\ndiabetic retinopathy. In DME there is collection of fluid in the critical\r\ncentral region of the retina, called as the \u201cmacula\u201d (the \u201cseeing\u201d portion of\r\nthe eye). In the second condition \u2013 Proliferative DR there are \u201cNew Vessels\u201d on\r\nthe retina. These new vessels are outgrowths from the normal vessels and appear\r\nin the retina due to lack of oxygen called as hypoxia. The problem with the new\r\nvessels is that their walls are very friable and they have a tendency to bleed.\r\nIf this stage is not treated it may lead to gross visual disability.<\/p>\r\n\r\n\r\n\r\n<p class=\"wp-block-paragraph\"><strong>Tertiary Prevention<\/strong> would be the late approach where complications\r\nhave reached a very critical stage and interventional procedures like laser\r\ntherapy and or surgical procedures are required to prevent the progression of\r\ndisease and stabilize vision. Laser therapy is a simple out-patient painless\r\nprocedure. If done in the proper way, the diabetic changes in the retina can be\r\ncompletely reversed. After the procedure, regular monitoring is necessary as\r\ndiabetic retinopathy symptoms may recur being a systemic disease. Timely\r\ntreatment can prevent up to 90% of vision loss from diabetic retinopathy.<\/p>\r\n\r\n\r\n\r\n<p class=\"wp-block-paragraph\">Other diabetes related eye problems are glaucoma\r\n(damage to optic nerve fibres), senile cataract due to ageing (clouding of the\r\nlens) and nerve palsies (affection of extra ocular muscles). People with\r\ndiabetes are twice likely to suffer from glaucoma than people without diabetes\r\nand risk increases with age and duration of diabetes.<\/p>\r\n\r\n\r\n\r\n<p class=\"wp-block-paragraph\">Thus, people with diabetes can prevent\/reduce\r\nthe occurrence of diabetic eye complications.
